How to get a PhilHealth number and ID for your first job
Get your PhilHealth number (PIN) through HR, online on the Member Portal or at a PhilHealth office, and what to fill in on the PMRF, from PhilHealth.

Your PhilHealth number, the PhilHealth Identification Number (PIN), is the one PhilHealth's own form calls "your unique and permanent number". You get it through your employer's HR, online through PhilHealth's Member Portal, or at a PhilHealth office. Here's how each route works and what to fill in, from PhilHealth's own pages and forms, read on 7 October 2026.
Registered before, maybe as a student or a dependent? PhilHealth's form says to "Always use your PIN in all transactions with PhilHealth", so give your employer that number instead of registering again.
How to get a PhilHealth number through your employer
This is PhilHealth's procedure for newly hired employees who don't have a PIN yet:
- Fill out two copies of the PhilHealth Member Registration Form (PMRF).
- Submit them to your employer's HR department.
- Wait for your Member Data Record (MDR) and PhilHealth ID card, which come to you through your employer.
Already have a PIN? PhilHealth says to report it to your employer, so they can put it on their ER2 form.
PhilHealth also says the premium is shared between you and your employer: your employer deducts your half from your monthly salary and sends the whole amount to PhilHealth.
How to get a PhilHealth number online
PhilHealth's Member Portal has an online registration page, the PhilHealth Online NHIP Registration for individuals, at memberinquiry.philhealth.gov.ph. From the portal's home page, click PhilHealth Member Registration.
Before you start, have two things ready as files you can upload:
- A filled-in and signed PMRF. You can download it from the Downloads page of philhealth.gov.ph.
- A supporting document, such as your birth certificate or a valid ID.
PhilHealth is strict about both. Its registration page says the PMRF and supporting documents have to be uploaded "for the system to properly validate and process the application otherwise, it will be disregarded."
Then enter an email address and the CAPTCHA, and follow the steps. PhilHealth says complete applications are processed "within three (3) to five (5) working days", and your PIN is emailed to you.
That page has two rules to know before you apply. An email address "can only be used once", and you can't submit more than one application. So use an email you'll keep, and don't apply twice if your PIN takes a few days.
How to get a PhilHealth number at a PhilHealth office
You can also walk in. PhilHealth's steps for members without an employer are to visit a Local Health Insurance Office (LHIO) or a PhilHealth Express outlet, fill out two copies of the PMRF and submit them there. Then you wait for your MDR and PhilHealth ID card.

What to fill in on the PMRF
The PMRF is a two-page form, and PhilHealth's own instructions on the back are short:
- Write everything in capital letters, and put "N/A" where a field doesn't apply.
- Use your name exactly as it appears on your birth certificate, and your mother's full maiden name as it appears there too.
- First time registrants attach a valid proof of identity.
- Tick Registration under Purpose, and pick a preferred KonSulTa provider near where you live or work.
- Sign over your printed name and write the date.

The form itself says "This form may be reproduced and is not for sale." Download it free from philhealth.gov.ph, and don't pay anyone for a copy.
After you get your PIN
Once you have your PIN, you can create a Member Portal account. The sign-up page asks for your PIN, name, birth date, sex, email and mobile number. PhilHealth's online services page says the portal lets you see your records and contributions, and view or print your MDR.
